A CUSTOMER ENGINE WAS RECEIVED AS AN EXCHANGE TO BE INSTALLED IN A AS35B3 HELICOPTER. A PRE-INSTALLATION INSPECTION FOUND SIGNIFICANT METAL CONTAMINATION IN THE OIL SYSTEM, INCLUDING THE FILTER AND MAG PLUGS. THE ENGINE WAS RECEIVED WITH AN 8130-3 INDICATING THAT A 800HR INSPECTION HAD BEEN COMPLETED (THIS INCLUDES A MAG PLUG INSPECTION.) SEVERAL OTHER DISCREPANCIES WITH THE DOCUMENTATION FOR THIS ENGINE WERE FOUND AND A SUP FORM 8120-11 IS BEING SUBMITTED.

DURING THE INITIAL START OF THE #2 ENGINE, IT WAS STARTING SLOW AND THEN THE TEMP TOOK OFF. I ABORTED THE START BEFORE ANY EXCEEDANCE. AFTER [tail no. removed] REACHED 0 AND THE REQUIRED WAIT TIME FOR THE STARTER I ATTEMPTED A 2ND START. THE #2 ENGINE WASN'T STARTING IN THE REQUIRED TIME, I WAS GETTING NO TOT RISE AND AT 15 SECONDS I ABORTED THE START AND CONTACTED MX. PREFORMED MULTIPLE STARTS WITH MX AND COULD NOT DUPLICATE THE ISSUE.

(CAN) DURING SCHEDULED INSPECTION, ENGINE FAILED TO START ON LT IGNITION SYS IAW TEST CRITERIA. IGNITER REMOVED AND FOUND ERODED, AND A SMALL ROUND PART FROM INSIDE THE IGNITER HAD MIGRATED TO THE TIP OF THE IGNITER AND BECOME STUCK AT THE END OF THE BODY. APPEARS TO BE A VERY HARD PIECE OF METAL THAT HAD IT BECOME DISLODGED FROM THE TIP OF THE IGNITER, IT POSSIBLY WOULD HAVE CAUSED SIGNIFICANT FOREIGN OBJECT DAMAGE TO THE TURBINE BLADES.

FOUND OIL LEAKING FROM SPLIT LINE OF COMPRESSOR MODULES 1 AND 2, ON NR 2 ENG CAUSED OIL INGESTION INTO BLEED AIR HEAT SYS, AFTER INSPECTING ENGINE IT IS DETERMINED THAT A MODULE 2-3 REPLACEMENT IS REQUIRED.

REMOVED DEFECTIVE PARTS AND FOUND METAL FROM RIVET SHAVINGS IN COMPRESSOR WASH NOZZLE. SUSPECT METAL MAY HAVE PASSED THRU COMPRESSOR WASH NOZZLE HOLE (0.054) TO COMPRESSOR AXIAL WHEEL OF ENGINE. SMALL EROSION FOUND ON COMPRESSOR WHEEL. THE PLUG WAS REPLACED AND THE EROSION FOUND WAS WITHIN LIMITS.
