THE BLADE WAS RECEIVED BACK FROM REPAIR AND INSTALLED WHERE IT AGAIN FAILED TO TRACK & BALANCE. THE REPAIR SHOWS THAT THE TRACKING FINGERS WERE REPLACED.

BONDING BRAIDS (P/N SA350A31185100) ARE PRONE TO BREAKING PREMATURELY AND ARE NOT LASTING AS EXPECTED. DESPITE PROPER INSTALLATION, THE COMPONENTS SHOW SIGNS OF FAILURE AFTER MINIMAL FLIGHT HOURS, INDICATING A POTENTIAL DURABILITY ISSUE.
(CAN) DURING VISUAL INSPECTION THE AME NOTICED A CRACK IN THE ADHESIVE BEAD OF THE STARFLEX ARM END BUSHING. FURTHER INVESTIGATION USING SOAP AND WATER (TO CREATE BUBBLES) CONFIRMED THAT THE END BUSHING WAS MOVING WHEN A LEADING/LAGGING FORCE IS APPLIED TO THE MAIN ROTOR BLADE, INDICATING THAT IT IS INDEED DEBONDED. DURING DAILY PREFLIGHT INSPECTION, NOTICED THAT THE TAIL ROTOR GEARBOX CONTROL LEVER LOWER BUSHING HAD BACKED OUT OF ITS HOLE A FEW THOUSANDTHS OF AN INCH. THIS IS A RECURRING PROBLEM.
DURING A PREFLIGHT INSPECTION, NOTICED THAT THE TAIL ROTOR GEARBOX CONTROL LEVER LOWER BUSHING HAD BACKED OUT OF ITS HOLE A FEW THOUSANDTHS OF AN INCH. THIS LEVER HAD BEEN REPLACED 82.5 HOURS PREVIOUSLY FOR THE SAME PROBLEM.

DURING REMOVAL IT WAS APPARENT THE OUTSIDE SPIDER SPLIT BUSHING WAS CAUSING THE ISSUE. REPLACED SPIDER UPON RECEIVING TOOLS & NEW ASSEMBLY.
AFTER DUAL FLIGHT CONTROL INSTALLATION PERFORMED FUNCTIONAL CHECK OF TAIL ROTOR CONTROLS FOR SMOOTHNESS OF OPERATION. NOTICED SOME BINDING WITH A SCRAPING SOUND. INVESTIGATED AND DETERMINED FORWARD BALL TYPE CONTROL CABLE WAS CHAFING AT FORWARD END WHERE ROD EXITS THE SHEATH. WILL REPLACE CONTROL CABLE WITH A NEW ONE.

(CAN) ON PERFORMING AN 600 HR INSPECTION ON THE STARFLEX 2 OUT OF 3 BUSHINGS WHERE FOUND LOOSE. THE BUSHING WHERE FOUND TIGHT TO HAND PRESSURE, BUT WHEN PUT IN A VISE THEY WHERE FOUND LOOSE. THE STARFLEX WAS REMOVED AND A NEW STARFLEX INSTALLED. THE OLD ONE CAN BE REPAIRED.

BATTERY TOTAL TIME 4 MONTHS & 400 HOURS. REPLACED STARTER GENERATOR FOR BENCH TESTING. NO DISCREPANCIES NOTED ON ELECTRICAL SYSTEM. STARTER GENERATOR HAD NO SIGNS OF HEAT OR DUST OR SMELL AND ROTATED SMOOTHLY. TEST RESULTS PENDING.
CREW DETECTED A STRONG ELECTRICAL BURNING SMELL DURING FLIGHT BACK TO BASE. NO SMOKE MADE A PRECAUTIONARY LANDING AT KCIC.
ON THE FIRST ENGINE START OF THE DAY THE STARTER WOULD NOT ENGAGE THE ENGINE. FOUND THE STARTER GENERATOR SHAFT HAD SHEARED.

DURING TAKEOFF THE PASSENGERS AND MYSELF SMELLED ELECTRICAL BURNING IN THE COCKPIT. RETURNED BACK TO THE AIRPORT. AFTER LANDING AND MAINTENANCE INSPECTION THE A/C COMPRESSOR LOCKED UP AND WOULDN'T TURN. TL-259861-2024 – REMOVED, REPLACED AND [S/N removed] THE A/C COMPRESSOR.
PILOT REPORTED SMOKE SMELL IN CABIN, TURNED OFF THE A/C SYSTEM AND SMELL WENT AWAY. LANDED AT BASE WITH NO FURTHER ISSUES. MECHANIC ON SITE FOUND WHERE A/C COMPRESSOR CLUTCH WAS INOP.

AIRCRAFT WAS FLYING AN EXTERNAL LOAD OPERATION WHEN THE PILOT NOTICED THE RED LOW HYDRAULIC WARNING IND LIGHT WAS ILLUMINATED. THE PILOT WAS ABLE TO SAFELY SET LOAD ON THE GROUND AND LAND BACK AT THE HELI-PAD. UPON VISUAL INSPECTION OF THE HYDRAULIC PUMP , THE DRIVE SPLINES WERE CHEWED UP AND THE BEARINGS INSIDE WERE ROUGH/ BINDING WHEN TURNED BY HAND.
PILOT PERFORMED A PRECAUTIONARY LANDING FOR #2 HYDRAULICS FAILED IN FLIGHT. MECHANIC DISCOVERED THE HYDRAULIC PUMP BELT HAD SHEARED. DURING REMOVAL IT WAS DISCOVERED THE HYDRAULIC PUMP BEARING FAILED AND CAUSED THE BELT TO SHEAR. MECHANIC REMOVED AND REPLACED THE HYDRAULIC PUMP PULLEY AND BEARING ASSY AND REPLACED THE HYDRAULIC PUMP BELT. BIS.
LOSS OF HYDRAULIC PRESSURE IN FLIGHT, EMERGENCY LANDING OCCURRED. AFTER INSPECTING HYDRAULIC SYSTEM FOUND HYDRAULIC PUMP SPLINES WORN AND NO LONGER FUNCTIONAL.

SIGNIFICANT QUANTITY OF HYDRAULIC FLUID FOUND LEAKING FROM RH HYDRAULIC SERVO. APPARENTLY LEAKING AT SHAFT SEAL. THERE WAS NOT, HOWEVER, A NOTABLE DROP IN FLUID LEVEL ON THE HYDRAULIC RESERVOIR SIGHT GLASS.

The expensive surprises on any used helicopter are missing logbooks, outstanding ADs, how much time is left before the manufacturer’s mandatory overhaul, and the same squawk written up again and again. Ask for the logbook entries that match each item. A repeated entry is a pattern, not a coincidence.
