DURING 100 HOUR LUBRICATION OF TAIL ROTOR DRIVE SHAFT SPLINES THE #3 TAIL ROTOR DRIVE SHAFT WAS FOUND CRACKED AT ONE ATTACHMENT EAR. TOTAL TIME OF PART IS UNKNOW AS IT IS AN ON CONDITION PART.
WHILE PERFORMING PREFLIGHT CHECK, PILOT NOTED THAT THE AFT MOST FLEXIBLE DISC PACK COUPLING ON THE TAIL ROTOR SEGMENTED DRIVESHAFTS AT THE INTERFACE WITH THE TAIL ROTOR GEARBOX WAS CRACKED. VERIFIED CRACKED COUPLING AND REPLACED WITH [S/N removed]. PN: 407-340-340-103
(CAN) AIRCRAFT WAS ABOUT TO DEPART WHEN THE TAIL ROTOR DRIVE SHAFT SHEARED ABOVE THE HORIZONTAL STABILIZER. NO INJURIES REPORTED AND ACFT RECEIVED LOCALIZED DAMAGE.

DURING ACCOMPLISHMENT OF THE APICAL LIGHTWEIGHT EMERGENCY FLOAT KIT (STC# SR01450LA) 3 YEAR INSPECTION THE SYSTEM IS ACTUATED AND THE EMERGENCY FLOATS ARE DEPLOYED. IT REQUIRED 3 ATTEMPTS TO ACTUATE THE SYSTEM. WE WERE UNABLE TO DETERMINE IF IT WAS A RIGGING ISSUE OR A LACK OF PHYSICAL TRAVEL OF THE CABLE AND HANDLE ASSEMBLY. THE FLOATS DID ULTIMATELY DEPLOY.
DURING A 3 YEAR INSPECTION OF THE EMERGENCY FLOAT GEAR SYSTEM THE INFLATION ACTUATION HANDLE BROKE, THE FLOATS DID NOT ACTUATE. TO CONTINUE THE INSPECTION THE ACTUATION CABLE WAS PULLED DIRECTLY, IT REQUIRED A LOT OF PULL TO INITIATE FLOAT INFLATION. ONCE INITIATED FLOATS INFLATED NORMALLY. SUSPECT THAT THE FLOAT RESERVOIR VALVE WHICH WAS DUE FOR OVERHAUL AT THE END OF THE MONTH WAS STUCK AND SURPASSED THE ACTUATION HANDLE'S ABILITY TO MOVE IT CAUSING IT TO SHEAR.
DURING 3 YR FLOAT SYS FUNCTION CHECK, PULL CABLE WAS ACTUATED AND PLUNGER CAME COMPLETELY OUT OF FLOAT VALVE BUT VALVE FAILED TO OPEN CAUSING TEST TO FAIL. MX REPLACED FLOAT BOTTLE.

DURING CRUISE, TRANSMISSION CHIP LIGHT ILLUMINATED. AT APPROXIMATELY 1310 IN CRUISE FLIGHT AT 1250FT ENROUTE TO EI11H, THE TRANSMISSION CHIP LIGHT ILLUMINATED. THE STUDENT FLYING RELINQUISHED THE CONTROLS TO ME. I MADE A MAYDAY CALL AND DECLARED AN EMERGENCY TO THE COMM CENTER, AND INFORMED THEM OF THE SITUATION. I IMMEDIATELY DIVERTED TO THE NEAREST AIRPORT, THAT BEING KPTN AND MADE A SUCCESSFUL PRECAUTIONARY LANDING WITH NO INJURIES OR DAMAGE TO THE AIRCRAFT.

A SPECIAL INSPECTION WAS INITIATED ON ALL OF THE OUTER COMBUSTION CASES IN THE FLEET IAW ROLLS-ROYCE OVERHAUL MANUAL M250-C47B 72-40-00. AIR LEAKAGE WAS FOUND BETWEEN THE TUBE ATTACH FLANGE AND THE OUTER COMBUSTION CASE BODY. THE OUTER COMBUSTION CASE WAS REMOVED FROM [S/N removed] AND A [S/N removed] OUTER COMBUSTION CASE WAS INSTALLED.
ENGINE REMOVED 12-25-18 DUE TO METAL IN OIL. DETERMINED CAUSE TO BE TURBINE NR 5 BEARING SPALLING. INSTALLED NEW BEARING. PART WILL BE RETURNED FOR EVALUATION ON SHIPPING NUMBER 4904011120191.
ENGINE REMOVED FOR NR 5 BEARING UPGRADE. REMOVED NR 5 BEARING DUE TO SPALLING ON OUTER RACE AT SPLIT. REPLACE WITH NEW.

AFTER TAKEOFF, ABOUT TO LEVEL OFF AT 1000 FEET, EXPERIENCED A CYCLIC CONTROL ISSUE. CYCLIC BINDING AND HEAVY CREEP REARWARD. HYD ACTUATOR BINDING.
(CAN) ON THE ACFT, THE DUAL CYCLIC CONTROL WAS MAKING NOISE WHILE MOVING IT LATERALLY, A CLICKING SOUND. AFTER INVESTIGATION, IT WAS FOUND THAT THE LOWER BEARING OF THE PIVOT ASSY WAS SLIGHTLY MOVING AXIALLY INSIDE OF ITS HOUSING. IT APPEARS THAT THE BEARING WAS NOT PROPERLY STAKED IN ITS POSITION. COPILOT CYCLIC PIVOT ASSY WAS REMOVED AND SENT TO HEADQUARTERS SHOP FOR BEARING REPLACEMENT.

(CAN) IT WAS REPORTED THAT THE BALL AND SPRING OF 1 OF THE SWASHPLATE GREASE FITTINGS LEFT THE FITTING AND LOGGED IN THE SWASHPLATE BEARING CAUSING A PREMATURE WEAR AND REMOVAL OF THE PART. BLACK MUD WAS COMING OUT WITH THE GREASE.

UPON INSPECTION OF THE AIRCRAFT THE TAIL ROTOR MAST NUT WAS FOUND IN A LOOSE CONDITION. SAFETY WIRE WAS STILL INTACT AND IN THE CORRECT ORIENTATION
(CAN) WHILE GREASING THE T/R GEARBOX PITCH CHANGE ASSEMBLY IT WAS NOTED THAT THE BOOT P/N 406-312-106-101 WAS BEING PRESSURIZED BY THE GREASING. REMOVED THE T/R GEARBOX PITCH CHANGE ASSEMBLY AS PER BELL MM. THE ASSEMBLY WAS DISASSEMBLED. IT WAS FOUND THAT THE BREATHING HOLE DID NOT ALIGN UP WITH THE SLEEVE BREATHING HOLE (PRESSURIZING THE BOOT. REMOVED THE SLLEVE AND REALIGNED THE BREATHING HOLES ON THE SHAFT AND THE SLEEVE FOR THE BOOT TO VENT AS PER BELL 407 CR&O AND MM. (TC# 20090608007)
IN FLIGHT, TAIL ROTOR PRODUCED A VIBRATING NOISE. TIGHTEN T/R PC LINK CAP NUT ONE FLAT. INSPECTED SYSTEM AND CHECKED WITH VIBRATION GEAR. EVERYTHING CHECKED OK. ALL WORK IAW MM. (K)

DURING THE PREFLIGHT INSPECTION ONE OF THE BEARINGS ON THE TAIL ROTOR PITCH CHANGE LINK WAS FOUND WORN. THE BEARING STARTED TO WEAR THROUGH THE OUTER RACE AND THE PITCH CHANGE LINK WAS CRACKED.
(CAN) WATER ENTRAPMENT CAUSED THE PEDAL STOP SOLENOID TO BECOME INOPERATIVE. WHEN THE LOWER WSPS PANEL WAS OPEN, WATER WAS TRAPPED IN THE AREA JUST BELOW THE PEDAL STOP SOLENOID DUE TO THE DRAIN HOLE BEING PAINTED OVER & OBSTRUCTED BY PAINT.
TAIL ROTOR YOKE PN 406-012-102-109, [S/N removed], HAS BEEN MFG INCORRECTLY. THE AREA WHERE THE FLAPPING STOP CONTACTS THE YOKE SHOULD BE MACHINED FLAT. THE YOKE IN QUESTION HAS TOO MUCH MATERIAL REMOVED, CAUSING A RIDGE TO BE FORMED AND RESULTS IN THE FLAPPING STOP NOT MAKING FULL CONTACT WITH THE YOKE. IN ADDITION TO THIS, THE YOKE IS BELOW MINIMUM THICKNESS DUE TO IMPROPER MACHINING. MFG HAS BEEN NOTIFIED AND THE PART HAS BEEN RETURNED FOR WARRANTY.
